Based on an observation made of the facility during the survey on 04/26/2022, and based on several attempts of testing the nurse call system. Patient room 5 does not report to the nurse call receiving station at the front nurses desk.

Based on an observation made during the survey on 04/26/2022, it was observed that the hallway in several areas is being used as general storage. The storage that is in the hallways and patient corridors currently does not allow the minimum 8 feet required in the patient hallway

Based on an observation made of the facility during the survey 04/26/2022 and based on an entire walk through of the facility, it was noted that several exit lights are not working in the hospital.

Fire Alarm System - Testing and Maintenance

Based on an observation made of the paperwork provided during the survey on 04/26/2022, the facility is not testing the fire alarm system in the correct intervals. The facility is missing 3 quarterly fire alarm inspections from 2019,2020, and 2021.

Sprinkler System - Installation

Based on an observation made of the facility on 04/26/2022 and based on a complete walk through of the building. During the walk through it was noted that the fire sprinkler head in the medication room in the ER area needs to be replaced. The fire sprinkler head is coated with foreign material.

Based on an observation made of the paperwork provided during the survey on 04/26/2022 it was noted that the fire sprinkler system is not being tested at the correct intervals. The facility is missing 3 quarterly fire sprinkler inspections from 2019.2020 and 2021.

Based on an observation made of the facility on 04/26/2022 and based on a walkthrough of the entire hospital, it was noted that there is no NFPA 701 tag on the curtains in patient room 2.

Building Services - Other

Based on an observation made of the facility on 04/26/2022 and based on a walkthrough of the entire building it was noted that the soiled utility room is being used as a janitors closet. It was also noted that special bathing room is now being used as storage facility. The storage in the special bathing room was up to the ceiling and the storage encompassed the entire room.

HVAC

Based on an observation made of the facility on 04/26/2022 and based on a complete walk through of the facility coupled with complete testing of all negative pressures in patient and staff restrooms, it was noted that the negative pressure was not working in the patent restroom across from the lab. It was also noted that the negative pressure was not working in patient room 4 restroom.

Based on an observation made during the survey on 04/26/2022 and based on a complete walk through of the entire hospital it was noted that storage is stored closer then 18" from the sprinkler head deflector in lab.

Based on an observation made of the paperwork provided during the survey on 04/26/2022 it was noted that the facility is missing 3 of the 4 quarters of fire drills for the entire facility in 2020 and 2021.

Based on an observation made of the paper work provided during the survey on 04/26/2022, it was noted that the facility is not doing the required annual receptacle testing of outlets.

Electrical Systems - Essential Electric Syste

Based on an interview with the maintenance staff during the survey on 04/26/2022, during that interview the staff mentioned that there are 2 generators on site, both are in service. In that same conversation it was noted that the staff member said that the generator for the old building was not in good shape. It was further noted that the staff mentioned that the generator was attempted to be replaced but the wrong size generator was brought on site. It was observed that the new generator is sitting on site not in service. The same staff member that talked about the old generator still in serviced mentioned his concerns about the functionality of the old generator that's still in service as of the inspection on 04/26/2022.

Based on an observation made of the paperwork provided during the survey on 04/26/2022 it was noted that the facility is missing several weeks and several monthly generator logs for 2020 and 2021.
