MAINTENANCE OF PHYSICAL PLANT

Based on observations, record reviews, and interviews, the facility failed to maintain the condition of the physical plant and overall hospital environment for the well being of patients. Findings include:

During the physical tour on 6/12/18 at 10:40 a.m., a ceiling with penetrations was observed in the facility's food prep area of the kitchen. Kitchen staff were observed actively preparing food on the counter below the penetrated ceiling.

During an interview with the Food and Dietary Director (Employee #19) on 6/12/18 at 10:40 a.m. in the kitchen area, Employee #19 stated he/she had not known when the ceiling had penetrations or how the ceiling penetrations occurred above the kitchen food prep area. In addition, Employee #19 stated he/she had contacted the hospital's maintenance department, but could not provide a work order or date of when the facility's maintenance was contacted. No further documentation provided.

During the physical tour on 6/13/18 at 11:00 a.m., layers of dust and unidentified black spots were observed on the return air vents on the facility's fourth floor, medical, surgical patient care hallway.

During the physical tour on 6/13/18 at 11:10 a.m., the ice machine grilles on the medical, surgical patient care unit had a white color mineral build up and the surface below the ice machine cavity was dirty.

During the physical tour on 6/13/18 at 11:20 a.m., condensation, and unidentified black substance markings were observed on the facilities heating and air ducts coverings inside the fourth floor mechanical room.

During the physical tour on 6/13/18 at 11:35 a.m., multiple unidentified dead bugs were found in a light fixture in medical, surgical patient room #4115.

During the physical tour on 6/13/18 at 11:40 a.m., multiple layers of dust were found on the air vent of medical, surgical patient bathroom #4021.

During the physical tour at 12:00 p.m., at 12:00 p.m., the ice machine grilles on the speech, physical therapy unit had a white color mineral build up and the surface below the ice machine cavity was dirty. In addition, a freezer in the physical therapy treatment room had layers of dust on the freezer's compartment holders. The freezer had multiple patient cold packs for therapy use.

During an interview with the facility's medical, surgical Registered Nurse (Employee #24) on 6/13/18 at 11:15 a.m. in the nourishment room, Employee #24 stated staff take turns cleaning inside of the refrigerators and cleaning the ice machine as needed.

During an interview with the Director of Nursing (Employee #2) on 6/13/18 at 11:15 a.m. in the nourishment room, Employee #2 stated Facilities Maintenance serviced the ice machines, but staff cleaned the refrigerators.

During an interview with the Environmental Services Director (Employee #25) on the medical, surgical patient care hallway on 6/13/18 at 11:45 a.m., Employee #25 stated environmental services staff were responsible for dusting air vents, and wiping external patient care use surfaces, including but not limited to countertops, refrigerators. Employee #25 further stated, environmental services staff were not responsible for removing bugs from light fixtures, but were expected to inform facilities maintenance if bugs were found in light fixtures.

A review of the facility's, "Patient Daily Room Cleaning" policy dated, 6/28/16, listed, Environmental Services ensured patient rooms were clean and disinfectanted with safe and appropriate methods.

During an interview with Facilities Maintenance Manager (Employee #26) on 6/13/18 at 3:00 p.m., Employee #26 stated the facility's maintenance department was responsible for servicing the functionality of the ice machines, but staff in the area, and/or environmental services staff were responsible for cleaning the external surfaces of the ice machines.
