Based on document review and interview, it was determined that the facility failed to maintain one of one diesel generator, affecting the entire facility.
Findings include:
1. Document review on April 5, 2022, at 1:32 p.m., revealed the annual fuel quality sample had not been completed.
Interview with the maintenance supervisor on April 5, 2022, at 1:32 p.m., confirmed the annual fuel quality sample had not been completed.
2. Document review on April 5, 2022, at 1:30 p.m., revealed the monthly conductance test had not been completed.
