PATIENT RIGHTS: RESTRAINT OR SECLUSION

Based on review of the hospital's policies and procedures, medical records, and interview, it was determined that the facility failed to require that nursing personnel adhere to the requirements for nutrition, hydration, and toileting when a patient is placed in seclusion and/or restraints for 1 of 1 patient (Patient # 4). This deficient practice poses a potential risk to the health, and safety of the patient when nursing personnel do not follow the hospital's policies/procedures when addressing a patient's needs for nutrition, hydration and toileting while the patient is in seclusion or restrained.

Findings include:

Policy titled "Seclusion and Restraint," revealed: "...Nutrition and Hydration: Patient needs for nutrition, hydration and toileting are addressed at a minimum with every 15 minute check...."

Medical Record dated 06/08/2020, for Patient #4, revealed that there was no documentation that the patient's needs for nutrition, hydration, or elimination needs were monitored and/or assessed.

Employee #4 confirmed during an interview conducted on 12/01/2020, that there was no documentation in the medical record for Patient #4's 15 minute checks while in seclusion for nutrition, hydration, or toileting.

Employee #1 confirmed during an interview conducted on 12/02/2020, that there was no documentation in Patient #4's medical record that the patient was offered nutrition or toileting while s/he was restrained and/or in seclusion.
