Based on record review, observation, and interview, the hospital failed to provide emergency laboratory services 24 hours a day. The lab was closed as of May 15, 2015 (13 of 13 days that included the day of this survey 05/28/15) and was not performing any lab testing within the lab. The glucometers used to test blood sugar of diabetic patients were removed from the patient care areas. A contract certified nurse aide (CNA) was conducting fingerstick blood sugars during the hours of 7 AM to 3 PM, Monday through Friday. No plan was indicated for providing glucometer readings when the CNA was not on duty.

Findings included:

Review of e-mails sent to the medical staff on May 15, 2015, by Personnel #4 on behalf of Personnel #7 and #8, indicated that the hospital ceased all laboratory testing in house and reporting of results. "...No testing may be done on the floors including Glucometers and Blood gases."

During a tour of the hospital on May 28, 2015, it was observed that the laboratory was not processing any lab samples. There were two employees in the lab but they were not processing any lab testing.

An interview with Personnel #4 on 5/28/2015, at 11:00 AM, confirmed that the lab was closed. "They ceased doing PT/INR on May 1, 2015 and ceased all in house testing and reporting as of May 15, 2015. This includes glucometers, iStat and blood gases. All glucometers and iStat machines have been removed from the patient care areas and are locked in the lab."

An interview with Personnel #6 on 5/28/2015, at 2:00 PM, documented that she had been performing glucometer finger stick blood sugars within the hospital under contract during the hours of 7:00 AM to 3:00 PM Monday thru Friday. She started providing this service under contract since 5/26/2015.
