The facility did not ensure that fire drills were held at unexpected times under varying conditions at least quarterly on each shift as required by the referenced LSC.

On 03/15/16 at 10:30 AM, the surveyor was not provided with documentation by the Director of Facilities that fire drills were conducted for the 1st quarter of 2016 and the drills for the 3rd shift 2015 were all silent drills conducted after 6:00 AM. The facility was utilizing the exception in the LSC 19.7.1.2 for silent fire drills between the hours of 9:00 PM through 6:00 AM not in compliance with the exception i.e. the drills were conducted at 6:05 and 6:10 AM.
