DISCHARGE PLANNING EVALUATION

Based on clinical record review and staff interview, it was determined the patient's post hospital care needs were not addressed in discharge planning for two (#2 and #3) of four (#2, #3, #8 and #10) patients. The failed practice did not assure that a discharge plan could be implemented upon discharge. The failed practice had the potential to affect all patients admitted to the Adult Unit. The findings were:

A. Patient #2 was admitted on 03/20/11. Review of the Adult Unit Care Coordinator Planning Form (no date) reflected on discharge there was an issue with medication cost. The Master Treatment Plan did not address. Review of the Social Service Progress notes revealed there was no evidence medication costs were addressed. Interview on 03/30/11 at 1408, the Clinic Director stated the Case Coordinator's notes were not in chart and they could address what you are asking about. At 1420, the Clinic Director presented the Case Coordinator's notes but they did not address how medications costs were being addressed. The Clinic Director confirmed there was no evidence the medication cost upon discharge was addressed and they needed to tighten up on documentation.

B. Patient #3 was admitted on 03/12/11. Review of the Master Treatment Plan dated 03/14/11 revealed in order to guide the treatment plan it was indicated the patient needed to have addressed "Finding suitable & (and) affordable housing". Review of the Social Service Individual Note dated 03/23/11 identified the patient was homeless. Interview on 03/30/11 at 1420, the Clinic Director stated he would get Case Coordinator's notes. Review of the Case Coordinator's notes revealed the patient's home situation was not addressed. Interview on 03/30/11 at 1530, the Clinic Director confirmed the clinical record did not address the patient's homelessness and suitable living arrangements upon discharge.
