Based on visual observation the facility failed to maintain sprinkler system devices through out the facility.. The deficient practice had the potential to affect 1 of the 3 smoke compartments and would allow for fire and smoke to travel from one compartment to another and could potentially affect 18 of the 18 patients.
Findings:
During facility tour on 12/01/2011 between the hours of 8:30am and 4:30pm, one sprinkler head in the therapy area by the bathroom and locker area was not covering concealed areas and would affect the coverage of the sprinkler. The sprinkler head was blocked by a small wall.

Based on visual observation the facility failed to provide proper ashtray containers. The deficient practice had the potential to allow smoke and fire back into the facilities corridors and means of egress and affect approximately 18 of the 18 patients of the facility.
Findings:
During facility tour on 12/01/2011 between the hours of 8:30am and 4:30pm, the ashtray containers in the smoking areas were not of the metal type with self-closing covers. The type that were present had a plastic or metal base with a plastic or metal tube with no self-closing lid.

Based on visual observation the facility failed to protect the kitchen deep fat fryer and provide instructions for the use of the portable fire extinguishers affecting 1 of 3 smoke compartments. This deficiency if not corrected could have the potential to affect 18 of the 18 patients in the dining room.
Findings: During tour of building on 12/01/2011 between the hours of 8:30am and 4:30pm and confirmed by the Director of Support Services the deep fat fryer did not have at least an 8" inch baffle separating or at least a 16"inch seperation between it and the open flame on the stove. Also, the facility had not provided a placard placed above each portable class- K fire extinguisher identifying its use as a secondary back-up means to the fixed automatic extinguishing system.
