Based on document reviews and interviews, the facility failed to ensure that each patient was informed, or when appropriate, the patient's representative (as allowed under state law), of the patient's rights, in advance of furnishing or discontinuing patient care whenever possible in ten (10) of ten (10) patient records. Records #1- #10
Finding:
On 10/22/2024 at approximately 12:00 PM, a review of ten (10) patient medical records was conducted. The ten (10) records contained no documentation that the patient had received their Patient Rights information.
On 10/22/2024 at 1:55 PM, during an interview with the Vice President of Compliance and Risk, the above finding was verified. She stated that the registration personnel, the ones responsible for giving the Patient Rights pamphlet to patients, were not documenting in the patient's medical record that the patients had received the Patient Rights pamphlet.
