Based on staff interview and a fire alarm activation simulation drill conducted by the Dover Fire Marshal Office, it was determined for 68 of 68 inpatients and for 39 of 39 outpatient program participants, the initiation of the building's fire alarm system by manual means was not in compliance. The facility used key-operated fire alarm boxes. Findings include:
The hospital policy entitled "Fire Plan" stated, "...Respond appropriately to fire conditions in a manner that will ensure the safety of patients, visitors and staff...Steps...Activate...accomplished by using your Fire Pull Key at the closet Pull Station..."
During a tour of the facility on 3/5/13 from 9:30 AM to 1:30 PM with Director of Environment of Care A, the following observation was made/acknowledged at the time of discovery:
A random sampling of the staff revealed that 3 out of 5 staff members could not provide the proper key to activate a selected key-operated fire alarm box.
