Based on visual observation the facility failed to assure that the automatic sprinkler system was installed to provide coverage for all areas in the building, in accordance with the Life Safety Code and NFPA 13. This deficiency could potentially affect 201 of 201 residents.
Findings:
During the facility tour and the record review, between the hours of 8:00am and 3:30pm, the contractor, who conducted the annual inspection of the system on 10/12/2010, made recommendations on the report. The contractor listed the following locations where an additional sprinkler head is needed to provide adequate coverage:
1. 3rd Floor - in the corridor by Room 339
2. 3rd Floor - in the corridor by Staff Lounge
3. 5th Floor - in corridor by Room 511

Based on visual observation and record review the facility failed to assure the commercial hood systems were maintained. The proper coverage from the suppression system, for the appliances protected under the hood, reduces fire emergencies. The deficient practice had the potential to affect 201 of 201 residents.
Findings:
During the facility tour and record review, between the hours of 8:00am and 3:30pm, the documentation from the semi-annual inspection of the hood suppression system showed an impairment. The technician had placed a yellow tag on the small hood system, on 11/29/2010, for having the wrong nozzles covering the grill. On the same date, a yellow tag had been placed on the large hood system for the wrong nozzles covering the range and fryers.
Additionally, it was observed that the area of the hood system, covering the griddles, had broken filters and missing expansion spacers.
