Based on observation and interview, the facility does not maintain all component of exit discharge, and does not have stair identification signs for two of two stairwell enclosures. (Lockport Division site)
Findings include:
During tour of the Lockport Division site on 6/22/15 at 11:00AM, it was observed that the outside steps, which are are a component of the exit discharge for the stair tower located at the South-East end of the hospital, were in disrepair. The steps were made of concrete; half of the middle step was missing, and the remaining half was very loose.
During tour of the Lockport Division site on 6/25/15 at 2:00PM, it was observed that the stair towers - located at the South-East and at the South-Center of the hospital - did not have signage inside the towers indicating the identification of the stair tower and the level of exit discharge. This finding was verified with Staff #7 (Maintenance) at the time of observation.
