Building Construction Type and Height

Based on observation components of the buildings designated construction type do not comply with, 19.1.6.2, and NFPA 220, 2012 Edition. This condition could affect individuals on the floor of fire incident from safely traveling the means of egress to the nearest exit.

The finding is:

On 12/01/2020 at 12:10pm while accompanied by the DF portions of the Hospital contain unprotected steel which does not comply with the minimum construction type requirements of 19.1.6.2. Location observed: Ground floor Loading dock at exterior dock a continuous horizontal gap exists between the wall and roof overhang. An unprotected steel beam can be viewed from this gap. This condition does not comply with an enclosure for the building's structural members.

Discharge from Exits

Based on observation, the facility failed to provide exit paths that are maintained as a continuously protected path to a public way. This deficient practice could affect staff and patients during egress due to a fire emergency evacuation from the building.

The finding is:

On 12/01/2020 at 11:30 am while in the company of the DF an exterior means of egress was observed in which the path is nonreliable. Location observed: exterior discharge path from Double South Stair leading through an exterior patio has no directional indication thereby it contains an unknown access to the public way. This condition does not comply with 7.7.1.1.

Hazardous Areas - Enclosure

Based upon observation, hazardous areas are not separated from the remainder of the occupancy and the means of egress. Failure to properly separate storage of combustible material (which represents a degree of hazard greater than that normal to the general occupancy due to quantity and density of materials) from required means of egress paths can compromise the safety of occupants if a fire were to originate at the stored material to block exiting.

A. On 12/01/2020 while accompanied by the DF hazardous areas located in sprinklered compartments were not maintained as fire and smoke proof enclosures to comply with 19.3.2.1 and 8.7.1. Location observed: Basement Trash Room #B-70A approximately 122 s.f. contains the following:

1. At 12:10pm entry door is a pocket door which does not comply with 7.2.1.8 for self closing, solid core swing door requirements.

2. At 11:50am perimeter walls were observed to have multiple openings one to the Dishwashing room the other to the Cafeteria. This condition does not comply with 19.3.2.1.2.


B. On 12/01/2020 while in the company of the DF hazardous areas located within non sprinkler protected compartments. These rooms were not maintained as as fire proof enclosures to comply with 19.3.2.1 and 8.7.1 and 8.3. Locations observed: Two Laundry rooms second floor contain designated fire resistant perimeter walls. These walls contain openings cut into them in order to accomodate flexible dryer ducts. Due to the opening the continuous protection and fire resistance rating of the walls is compromised.

Corridors - Areas Open to Corridor

Based upon observation, waiting areas open to the corridor are not provided with supervision or smoke detection system. Failure to provide supervision or smoke detection devices may prevent the building's occupants from being alerted to a fire related emergency. This deficient practice could affect the safety of patients, staff, and visitors.

The finding is:

On 12/01/2020 while in the company of the DF, it was observed that the second floor Waiting Area for the Outpatient Services is open to the corridor and lacks a supervisory station which is able to observe all locations or smoke detection devices to comply with 19.3.6.1.
