Based on observation and staff interview, the facility failed to provide exit access that is arranged so that exits are readily accessible at all times. The deficient practice renders the exiting obstructed and impeded to a full instant use, affecting egress traffic from one office/library. Census at the time of the inspection is unknown.

Findings Include:

During the complaint inspection conducted on 11/21/2012 between 11:00 AM and 3:00 PM the following determination is made:

1) It is determined that the complaint was due to temporary construction barriers being constructed in the corridor outside of an office/library staffed with three employees. Based on investigation, it is determined that the issue resulted out of a failure in construction coordination. The hospital directed the contractors to begin work prior to the office/library being relocated. After conversation with hospital administration and State Facilities Management on-site inspector, the temporary walls were removed and are stated to remain cleared until such time that the office/library has been relocated. An email was received at 2:49 PM from the hospital environment of care interim administer stating, "I have just returned from the Unit 15 area. The contractor has removed the section of the temp. wall adjacent to the subject door. They also returned the temp wall back to the original wall to prevent any access behind the temp wall. I allowed the contractor to leave the top temp. wall track in place to provide stability to the area. There is no impediment to egress from the Neuro Library." This was verified by the State Facilities Management on-site inspector via phone conversation.
