Based on document review and interview, the facility failed to include all presenting patients on the EMTALA log in 1 (patient 1) of 17 patients.
Findings Include:
1. Surveyor observed lack of documentation of patient 1's name/description in the EMTALA log.
2. Interview on 01/08/24 at 1:37 pm with P50 (Director of Risk and Performance Improvement) and P51 (Chief Executive Officer) remember patient in question (patient 1) being at facility - transferred from other facility per ambulance.

Based on document review and interview, the facility failed to provide a medical screening on 1 (patient 1) of 2 patients transferred to another facility.
Findings Include:
1. Review of policy titled: EMTALA last reviewed 01/2023, indicated that any person who presents to the hospital will be evaluated to determine whether the person has an emergency condition: The provider is responsible for completing a medical screening.
2. Patient 1 did not have a medical record; lack of documentation of completed medical screen.
3. Review of medical record for facility #2 indicated patient 1 presented there on 12/1//23 via ambulance after being taken to facility #1 and refused care due to pregnancy.
4. Interview on 01/08/24 at 1:35 pm with P51 (Chief Executive Officer) confirmed lack of signed request for transfer and lack of medical screening or completion of the EMTALA paperwork or any documentation that patient in question was at facility on 12/11/23.
5. Interview on 01/08/24 at 1:37 pm with P50 and P51 remember patient in question (patient 1) being at facility - transferred from other facility per ambulance.
