Based on observation and record review, the facility failed to provide a fire alarm system that was properly tested in accordance NFPA 70 and NFPA 72 for 1 of 1 fire alarm systems. This could potentially affect 13 of 13 patients in the facility at the time of survey on the event of a smoke or fire emergency and the fire alarm system did not properly function as designed.
Findings:1) During a check of the Fire Alarm Control Panel on 10/31/11 at 1:00pm, the panel was observed to have an annual certification tag affixed to it dated 6/1/2010 by Century Link Securities.
2) Review of the fire alarm paperwork in the facility maintenance department revealed no other documentation could be produced showing the Annual Certification had been conducted since 6/1/2010.

Based on document review, the facility failed to provide Generators that are exercised under load for 30 minutes per month in accordance with NFPA 99 for 1 of 1 Emergency Generators in the facility. This could potentially affect 13 of 13 patients in the facility in the event of a power failure to the Hospital and the emergency generator failed to provide emergency power as designed.
Findings: During a review of the facility paperwork n 10/31/11 at 2:30pm, the facility could provide no documentation that monthly load tests have been conducted on the emergency generator in the past year.
