Based on surveyor review of the facility's policies & procedures and staff interview on 12/8/16 at approximately 9:30 A.M., the facility failed to provide a "Risk Assessment Procedure" in accordance with Chapter 4 of the 2012 Edition of NFPA 99; Health Care Facilities Code.
The facility administrator was unable to produce a "Risk Assessment Procedure" performed by a qualified Personnel.
