Based on observation and interview the dish washing machine is not maintained in good repair and is not operated according to manufacturer's instructions.
Findings include:
On 7/3/2012 observation of the dish machine while in use revealed the front sprayers (pre-wash sprayers) were not functioning.
Interview with Staff #1 on 07/03/12 revealed that the motor that powers these sprayers had been removed the Thursday prior to the onsite investigation. A timetable for replacement had not been established at that time.
This finding was verified with Staff #3 on 07/03/12.
