Based on observation during the course of the survey conducted on May 15, 2015, it was determined the facility failed to maintain the doors that protect corridors in accordance with NFPA 101. The following evidenced this:
During the walk-through with the Maintenance Supervisor, Facilities Director and Administrator:
1. The doors to patient room 124 and patient room 216 did not close and positively latch into the frame.

The maintenance staff acknowledged the above items during the survey.

During the walk-through with the Maintenance Supervisor, Facilities Director and Administrator:
1. The following areas were not equipped with self closures; over 50 square feet in size and utilized by the facility for storage of combustibles.
a. Room 115
b. Fire door at Pantry
c. Fire door at Old Surgery Section
The maintenance staff acknowledged the above items during the survey.
