The hospital's "Consent to Treatment, Guarantee of Payment, Assignment of Benefits, Protected Health Information, Consent to Care Notice", which contains patient rights, last approved 6/2021, states, in part "I have read this form, or it has been read to me, and I understand it"...

On 2/8/2023 at 10:15 AM, Patient #6's medical record was reviewed with the Director of Quality. The hospital failed to provide evidence that Patient #6 received a copy of the "Consent to Treatment, Guarantee of Payment, Assignment of Benefits, Protected Health Information, Consent to Care Notice", which contains patient rights, nor any evidence of a signed acknowledgement.

On 2/8/2023 at approximately 10:25 AM, this finding was confirmed by the Director of Quality at the time of the review.

On 2/8/2023 at 10:15 AM, Patient #6's medical record was reviewed with the Director of Quality. The hospital failed to provide evidence that Patient #6 received a copy of the "Consent to Treatment, Guarantee of Payment, Assignment of Benefits, Protected Health Information, Consent to Care Notice", which includes patient rights, nor any evidence of a signed acknowledgement.
