Based on findings from document review and interview, the facility failed to ensure that Security Staff, who apply restraint or seclusion, monitor and access patients in restraint or seclusion, received training in First Aid training and certification in the use of Cardiopulmonary Resuscitation (CPR). These failures place patients at risk for potential harm.

Based on findings from document review and interview, in 2 of 2 Security Officer's personnel files reviewed, both lacked evidence of training in First Aid and certification in the use of Cardiopulmonary Resuscitation (CPR). Additionally, 22 of 32 Security Staff presently employed by the hospital lacked certification in CPR and (32 of 32) Security Staff lacked First Aid training. This lack of training could place patients at risk for potential harm.
Findings include:
-- Per review of personnel files for 2 Security Officers (Staff A and Staff B, both hired 6/2012) both lacked documentation of First Aid training and CPR certification.
-- Per review of the facility's policy and procedure (P&P) titled "CPR for Clinical Staff" last revised 4/2016, it indicated Security Staff are required to be certified in CPR. However, review of P&Ps did not indicate Security Staff are required to have First Aid training.
-- Per interview of Staff A on 3/2/17 at 9:40 am, and Staff C (Security Officer) on 3/2/17 at 3:30 pm, both revealed they do not have First Aid training or CPR certification.
-- During interview of Staff D (Director of Quality) on 3/2/17 at 2:30 pm, he/she acknowledged the above findings.
