Based on observation and staff interview the facility failed to assure that the separation wall to a non compliant building was properly sealed and secured. This may cause the spread of smoke and fire to the adjacent buildings. This will affect 3 out of 3 smoke zones. This facility is licensed for 18 with a census of 3 residents at the time of the survey.

FINDINGS INCLUDE:

During the tour on 7/27/10 from 11 AM to 3 PM the following was found:

1.) The 3 hour fire doors located by the central supply did not latch when released.
2.) The North lobby doors to the clinic did not latch when released.

Staff "M" was present and aware of the finding.

Based on observation and staff interview the facility failed to assure that corridor doors close tightly to prevent gaps, allowing for the spread of smoke and fire. This will affect 3 out of 3 smoke zones. This facility has a capacity 18 with a census of 3 residents at the time of the survey.

FINDINGS INCLUDE:

During the tour on 7/27/10 from 11:00 AM to 3:00 PM the following was found:

1.) The 3 hour fire doors located by the central supply did not latch when released.

Based on observation and staff interview the facility failed to assure that corridors are clear and unobstructed. This may delay egress from the hospital through this particular exit path. This will affect 1 out of 3 smoke zones. The hospital has a capacity of 18 beds with a census of 3 at the time of the survey.

FINDINGS INCLUDE:

During the tour on 7/27/10 from 11:00 AM to 3:00 PM it was observed that the corridor to the clinic has a round table in the corner for displaying pamphlets which was obstructing exiting.

Based on observation, record review and staff interview the facility failed to assure that the generator is properly tested. This deficient practice may leave the facility in total darkness. This deficiency affects 1 of 3 smoke zones. This facility has a capacity of 18 beds with a census of 3 at the time of the survey.

FINDINGS INCLUDE:

During the tour on 7/27/10 from 11:00 AM to 3:00 PM it was found that the generator is rated at 376 amps and should be tested monthly at 30% of its load capacity which is 113 amps. The generator log shows the average test is at 95 amps.
