Intakes: TN00030192

Based on medical record review and interview, it was determined the facility failed to ensure the family was notified for 1 of 1 (Patient #4) sampled patients with documented accidents and/or incidents.

The findings included:

1. Medical record review for Patient #4 documented an admission date of 3/12/12 to the psychiatric unit of the hospital.

Review of facility incident reports documented the following:
3/16/12- Patient #4 was hit by his roommate on the left cheek and had a laceration. There was no documentation the family was notified of the incident or injury.

4/17/12- Patient #4 had a fall in his room near the bed and sink. There was a moderate injury with bandaging to the patient. The family was not notified of the incident or injury.

4/19/12- Patient #4 had a fall between the bed and the bathroom. No injuries were documented. The family was not notified of the fall.

4/24/12- Patient #4 fell with laceration and was sent to the Emergency Department for stiches. The family was not notified of the fall or injury.

2. During an interview in a room on the psychiatric unit on 9/5/12 at 11:55 AM, the Director of the unit stated, "It should be documented that the family was notified." She further stated she would expect all attempts to reach the family documented in the medical record.
