Based on record review and staff interviews, the facility failed to assure that the sprinkler system is maintained and tested in accordance with the NFPA 13 and NFPA 25. This deficient practice fails to ensure that the sprinkler system will operate properly in the event of a fire, affecting all 12 patients and all occupants in 4 of 4 smoke zones. The facility has a capacity of 12 with a census of 7.
During the tour conducted on 1/13/2016, between 12 pm and 3:00 pm, the following is observed:
1.) At approximately 1 pm it is discovered that there are no sprinkler heads installed in the Kitchen Walk-in Coolers.
Maintenance Staff A was present and acknowledged the finding.
NFPA Standard: All automatic sprinkler and standpipe systems required by this Code shall be inspected, tested, and maintained in accordance with NFPA 25, Standard for the Inspection, Testing, and Maintenance of Water-Based Fire Protection Systems. 2000 NFPA 101, 9.7.5.

Based on observation and staff interview, the facility did not ensure that electrical wiring and equipment is installed and maintained in accordance with NFPA 70, National Electrical Code. This deficient practice does not ensure prevention of an electrical fire or electric shock hazard, affecting 0 patients and all occupants in 1 of 4 smoke zones. The facility has a capacity of 12 with a census of 7.
During the tour conducted on 1/13/2016, between 12 pm and 3:00 pm, the following is observed:
1.) At approximately 1:45 pm it is discovered that there is an extension cord being used as permanent wiring in the Kitchen to a radio.
Maintenance Staff A was present and acknowledged the finding.
NFPA Standard: Electrical wiring and equipment shall be in accordance with NFPA 70, National Electrical Code, unless existing installations, which shall be permitted to be continued in service, subject to approval by the authority having jurisdiction. 2000 NFPA 101, 9.1.2
