Based on observations during the tours of 3 South, the Dual Unit and 2 South from 8:15 A.M. to 8:45 A.M. on 2/3/14 and staff interviews the Hospital failed to have completed the repairs to countertops and flooring in the treatment room, on the 2 South Unit as of 1/24/14. A new observation of peeling paint was seen in the shower stalls on the Dual Unit.

Findings include:

The Facilities Supervisor was interviewed at 7:45 A.M. on 2/3/14. The Facilities Supervisor said the attempts to correct the missing Formica countertop edging in the treatment room on 2 South were unsuccessful and the new flooring was ordered but not yet installed.

The Surveyor observed during the tours of 2 South and 3 South Unit, on 2/3/14 from 8:15 A.M. to 8:45 A.M. that the Formica around the hand washing sink cabinetry was still broken, the floor tiling remained cracked and was still missing pieces.

During the tour of the Dual Unit the Surveyor observed that paint was peeling off the walls of 4 shower stalls.

The Facilities Director said the shower stall walls are going to be repaired/replaced with a material that was called ultra white rock. The work was not yet scheduled because structural work and plumbing needed to be completed.
