Based on observation and interview the facility failed to provide a COVID-19 Infection Control Screening process for all individuals prior to entering the facility. This failed practice could expose patients and staff in the facility to exposure to COVID-19.
The findings are:
A. On 11/19/2020 at 9:00 am during observation of Emergency Room Entrance (only entrance to facility), there is no observable screening station and this surveyor along with 2 other people entered the facility with no COVID-19 screening conducted.
B. On 11/19/2020 at 9:05 am during interview:
1) S#4 (Administrator) confirmed the only entrance to the facility open to the public is the Emergency Room Entrance and there is not a screening station at that entrance.
2) S#4 confirmed patients coming into the facility are screened by registration when they sign in.
3) S#4 confirmed COVID positive person could enter the facility without screening and could potentially expose patients and staff to COVID-19 due to lack of screening at the door.
C. On 11/19/2020 at 9:30 am during interview:
1) S#2 (Director of Nursing) confirmed there is no screening done at the entrance to the facility and this practice could increase risk of exposure within the facility.
2) S#2 confirmed this issue would be addressed today.
