Based on observation and interview, it was determined the facility is a two-story building and consisted of Type II construction. The facility failed to assure minimum building construction requirements were maintained. This facility has 25 certified beds and at the time of the survey the census was 12.
Findings include:
Observation and interview on 5/28/15, revealed a hole (approximately one foot in size) in the ceiling of the OB Kitchen. Maintenance Staff A verified this observation.

Based on observation and interview, the facility is not ensuring that doors to patient rooms, offices and other ancillary areas are free of impediments that would prevent the doors from being closed or that the doors are provided with suitable hardware that keeps the doors shut tightly into their frames. 1. Observation and interview on 5/28/15, revealed the door to the South Hall Med Surge Shower Room (2A510) failed to close and latch properly when tested.
2. 2. Observation and interview on 5/28/15, revealed the door to the West Hall Med Surge Linen Room (2A214) failed to close and latch properly when tested.

Based on observation and interview, the facility failed to assure that stairways used in exits are in accordance with Life Safety Code 7.2. This facility has 25 certified beds and at the time of the survey the census was 12.
Findings include:
Observation and interview on 5/28/15, revealed kitchen supplies/equipment and holiday decorations being stored in the 1st Floor North Exit (from main stairwell). Maintenance Staff verified this observation.

Based on observation and interview, the facility failed to maintain a complete automatic sprinkler system in accordance with National Fire Protection Association (NFPA) 25, 1998 edition. This facility has 25 certified beds and at the time of the survey the census was 25.
Findings include:
1. Observation and interview on 5/28/15, revealed a missing escutcheon ring in Outpatient Room 1A204.
2. Observation and interview on 5/28/15, revealed a missing escutcheon ring in Outpatient Workroom.
3. Observation and interview on 5/28/15, revealed a missing escutcheon ring in Beauty Room 2A515.
4. Observation and interview on 5/28/15, revealed a missing escutcheon ring in the corridor outside of the Main Housekeeping Room.
