Based upon review of the facility's policies and procedures, clinical record review and staff interviews, it was determined that the hospital does not ensure that services are provided in a safe and effective manner that permits the hospital to comply with all applicable rules and standards. The requirement is not met as follows:care in a safe setting.
Findings:
Based on medical record, policies and procedures review and staff interviews, nursing staff failed to protect patient #1 from potential injury during MRI scan by ensure patient did not have any electrodes or monitor during MRI scan, patient #1 sufferred third degree burns. Nursing staff failed to comply with policy Consent MRI Scan, number1200.102.250.24 which states under Section Nursing Unit Responsibility paragraph B subparagraph 2 have patient remove all metallic/metal objects (any and all metal must be removed prior to the MRI scan). Patient #1 received MRI scan with electrodes and monitor were still attached on body. Nurse and MRI tech did not ensure patient had removed all electrodes and monitor prior to MRI scan.
Interviwed with staff #5 on March 8, 2010 at 1:45 p.m. in the facility conference room stated "that 93% of the time nurses removed monitor and other times the MRI tech is the one removing monitor.
