Based on interview, record review and review of policies and procedures, the hospital failed to ensure a patient had a physician order for mental health counseling for 1 of 1 patient records reviewed (Patient #1).

Failure to have a physician's order puts patients at risk from poor outcomes when the treating physician is not involved in the mental health treatment.

Findings included:

1. Review of Patient #1's medical record revealed:

a) The patient did not have a physician's order to receive mental health counseling.

b) Patient #1 had a history of anxiety and depression. The patient was seen on 4 separate occasions for Behavioral Health Services starting in 07/01/19 and ending 08/05/19.

2. On 10/24/19 at 10:00 AM, the investigator interviewed a licensed social worker (Staff #1). Staff #1 stated they did not inform the physician and the clinical nurse manager that the patient had requested some mental health counseling for their anxiety and depression.

3. On 10/24/19 at 11:00 AM, the investigator interviewed the Chief Clinical Officer (CCO). The CCO stated that all services a patient receives in the hospital need a physician's order.
