COVID-19 Vaccination of Facility Staff

Based on document review and interview, the facility failed to vaccinate for COVID-19 or issue medical/religious exemptions to equal 100 %.
Findings include:
1. Review of facility administrative documents indicated the staff COVID-19 vaccination percentage for facility was 82.5%.
Total staff = 8,573
# of staff with completed series of vaccine = 6290 (73.4%)
# of staff completed first series of vaccine = 298 (3.5%)
# of religious exemptions = 478 (5.6%)
# of medical exemptions = 8 (0.1%)
# of noncompliant = 1499 (17.5%)
2. Interview with S4, Director of Operations, on 5/23/22 at approximately 12:30 pm, indicated the facility is working hard toward compliance, also indicated the remote workers were included in the numbers and facility working with contracted vendors to achieve compliance as well.
