Doors with Self-Closing Devices

Based on observation and staff interview on 11/28/17, it was determined the facility failed to ensure that all self-closing doors are maintained operational and in good repair.
Findings include:
1. During a tour conducted at 10:30 AM, in the presence of Staff #40, the door to Support Room #T4502 would not close properly when operated and would not prevent the transmission of smoke to the exit corridor in the event of a fire.
2. This finding was confirmed by Staff #40.

Based on observation and staff interview on 11/28/17, it was determined the facility failed to ensure that a fire-safe environment is maintained for patients, staff and the general public.
Findings include:
1. During a tour conducted at 10:30 AM, in the presence of Staff #40, it was found that within Todd #4, PACU stretchers, medical equipment and other supplies were being held in storage without being separated and contained in a room with smoke resisting partitions and doors.
2. This finding was confirmed by Staff #40.
