1. Based on generator log review and interview, it was determined the facility did not meet Life Safety Code standards related to emergency generator inspection and line isolation monitor testing. The failed practices had the potential to affect all patients admitted to the facility. See Tag K-130 and Tag K-144.

2. Based on observation and interview, the facility failed to ensure outdated medications were not available for patient use for all four nursing medication areas of the hospital. The potential existed for out of date medications to be administered to patients in all four nursing areas of the hospital. Findings follow:
A. A tour of the facility was conducted on 06/09/10 between 1400 and 1530. The following medications were observed to be out of date and available for patient use:
Operating Room Anesthesia Cart
Five Hydralazine 20 milligrams(mg)/milliliter (ml) expired 05/10.
Procedure Room
1) Two Bacteriostatic 0.9% Sodium Chloride expired 03/01/10;
2) Two Bacteriostatic 0.9% Sodium Chloride expired 05/01/10; and
3) One Xylocaine 1% with Epinephrine expired 03/09.
Post Anesthesia Care Unit
1) Two Heparin 50 units/ml 500 ml expired 06/01/10; and
2) Twenty-four Demerol 50 mg/ml expired 05/01/10.
Medical/Surgical Unit
Four Cenolate 1000 mg/2 ml expired 03/01/10.
Emergency Room
1) Two Fosphenytoin Sodium 2 ml expired 04/01/10;
2) Three Gentamycin 30 mg/2 ml expired 05/01/10;
3) One Prochlorperazine 10 mg/ ml expired 10/09; and
4) Two Apresoline 20 mg/ml expired 05/10.
B. Interviews were conducted with the Operating Room Charge Nurse and the Director of Nursing/Administrator on 06/09/10. Interview at 1440, the Operating Room Charge Nurse verified the medications in the Operating Room Anesthesia Cart and Post Anesthesia Cart were out of date. Interview at 1515, the Director of Nursing/Administrator verified the medications in the Medical/Surgical Unit and Emergency Room were out of date.
