NURSING CARE PLAN

Based on observations, interview, and review of hospital policies and procedures, the hospital failed to ensure that the patient's plan of care was initiated and implemented as soon as possible after the patient's admission to the hospital for 3 of 16 patient charts reviewed for care and services. (Patient 2, 9, and 10)


The findings are:


On 01/14/15 at 1:30 p.m., review of Patient 2's chart revealed the patient was admitted on 1/3/15. Review of the patient's plan of care revealed the plan of care was initiated on 1/11/15 which was eight (8) days after the patient's admission to the hospital. The finding was verified on 01/14/15 at 1:35 p.m. by Clinical Specialist 2.

On 01/16/15 at 1:25 p.m., review of Patient 9's chart revealed the patient was admitted on 11/17/14. Review of the patient's plan of care revealed the care plan was initiated on 11/20/14 which was three (3) days after the patient was admitted. The finding was verified on 01/16/15 at 2:57 p.m. by the Director of Quality.

On 01/16/15 at 1:55 p.m., review of Patient 10's chart revealed the patient was admitted on 12/23/14. Review of the patient's plan of care revealed the plan of care was initiated on 12/27/14 which was four (4) days after the patient was admitted. The finding was verified on 01/16/15 at 2:58 p.m. by the Director of Quality.
