Based on observation and staff interview the facility fails to ensure doors between smoke zones resist the passage of smoke. This deficient practice could result in smoke spreading between smoke zones affecting 2 of 7 smoke zones. The facility has a capacity of 10 and a census of 3 at the time of this survey.
Findings Include:
On 05/13/2010 between 10:00 am and 3:00 pm the following was observed:
1. The smoke barrier doors between post-op and patient rooms have a gap due to missing astragal.
Staff A was present and confirmed the doors have a gap. Staff B stated the astragal was removed to be replaced with a new one.

Based on observation and staff interview the facility fails to ensure the means of egress is continuously maintained free of obstructions and impediments. This deficient practice could result in delayed egress in the event of and emergency affecting 1 of 7 smoke zones. The facility has a capacity of 10 and a census of 3 at the time of this survey.
Findings Include:
On 05/13/2010 between 10:00 am and 3:00 pm the following was observed.
1. Furniture was observed in the patient room corridor.
Staff A was present and confirmed the furniture was in the corridor.
