Based on document review and staff interview it was determined the facility failed to maintain the automatic sprinkler system in accordance with National Fire Protection Association (NFPA) 25. Findings include:
1. On 12/15/10 at approximately 9:00 a.m., a sprinkler system inspection report dated 11/05/10 and prepared by Brewer and Company was reviewed. This inspection report indicated that the building 1 and 1/4 inch test and drain valves are leaking and need to be changed.
2. On this same date and time, an interview with the assistant director of support services revealed that the valves had not been repaired as of this date 12/15/10.

a. All receptacles and fixed equipment within the area of the wet location shall have ground-fault circuit-interrupter protection for personnel if interruption of power under fault conditions can be tolerated, or be served by an isolated power system if such interruption cannot be tolerated.
This Standard is not met as evidenced by:
Based on observation and staff interview it was determined the facility failed to maintain all electrical wall receptacles in accordance with National Fire Protection Association (NFPA) 70. Findings include:
1. On 12/14/10 at approximately 3:00 p.m., two (2) electrical outlets in the operating room area (soiled utility room and anesthesia work room) near hand sinks were observed not to be ground fault circuit interrupter (GFCI) receptacles.
2. An interview with the assistant director of support services on 12/15/10 at approximately 10:00 a.m., confirmed that the two (2) aforementioned receptacles were not ground fault protected.
