Based on review of documentation the Hospital failed to ensure completed documentation for 3 of 10 surgical records reviewed.
Findings include:
Review of policies and procedures related to positioning the patient for surgery included the selection of the patient position was made in collaboration with the surgeon and anesthesia provider based on the surgical procedure. Documentation should include positioning devices that include but were not limited to: speciality tables/beds, table extensions, arm and footboards, gel and foam pads and stirrups.
Review of the Nursing Operative Record indicated it was a preprinted form with a section to indicate the type of OR table used.
Review of Patient #1's, Patient #8's and Patient #9's Nursing Operative Records indicated the section regarding the type of OR table was blank.
