Based on observations the facility failed to maintain the fire alarm system in operable condition.

Findings Include:

On December 9, 2014 at 1:30 p.m. it is observed that there is a supervisory trouble alarm present on the fire alarm enunciator panel in the main lobby indicating an unknown issue at building "9".

Based on observations the facility failed to maintain the automatic sprinkler system or its components in reliable operating condition.

Findings Include:

1.) At 1:45 PM on December 9, 2014 It is noted during document review that 9 sprinkler heads were identified as having been painted by the automatic sprinkler service company. The Director of Maintenance states that these heads were cleaned by a different contractor.

Pursuant to section 9.7.1.1 of the 2000 Edition of the NFPA 101 Life Safety Code, facilities shall maintain automatic sprinkler systems in accordance with NFPA 13 (1999 edition).
In accordance with NFPA 13 (1999 edition) section 3-2.6.3* "Unless applied by the manufacturer, sprinklers shall not be painted, and any sprinklers that have been painted shall be replaced with new listed sprinklers of the same characteristics, including orifice size, thermal response, and water distribution."

2.) Between 2:00 PM and 3:30 PM it is observed throughout buildings 6 and 7 that fire alarm and data wires are hanging from, and attached to, sprinkler piping above the level of the ceiling.

Pursuant to section 9.7.1.1 of the 2000 Edition of the NFPA 101 Life Safety Code, facilities shall maintain automatic sprinkler systems in accordance with NFPA 13 (1999 edition).
In accordance with NFPA 13 (1999 edition) section 6.1.1.5* "Sprinkler piping and hangers shall not be used to support non-system components."
