Based on observation and interview, it was determined the facility exceeded maximum allowable story height for this type of construction.

Findings include:

1. Observation on November 17, 2021, at 9:35 a.m., revealed the facility exceeded the maximum allowable story height for three story, Type II (000), unprotected, noncombustible building.

Exit interview with facility representative #1 on November 17, 2021, at 12:15 p.m., confirmed the facility exceeds maximum allowable story height for this type of construction.

Based on observation and interview, it was determined the facility failed to maintain portable fire extinguishers, affecting one of approximately ten fire extinguishers within the component level.

Findings include:

1. Observation on November 17, 2021, at 10:15 a.m., revealed the flush mounted fire extinguisher located in the 2nd floor Short Procedure Unit area lacked adequate signage.

Based on observation and interview, it was determined the facility failed to maintain smoke barrier doors, affecting two of five smoke compartments.

Findings include:

1. Observation on November 17, 2021, at 11:00 a.m., revealed the smoke barrier doors located on the 3rd floor had unsealed penetrations in the door frame.
