Based on interview and record review the facility failed to have a registered nurse evaluate the nursing care of each patient in that 6 of 7 (Patients #6, #7, #8, #9, #11, and #12) patients that were on a ventilator did not receive oral care every 4 hours per facility policy.

Findings included:
The following is a representative sample of two patients that did not receive oral care every 4 hours per facility policy after fly larvae were found in 2 patients that were on ventilators on 08/24/21.
Patient #12 was admitted on 08/01/21 with a diagnosis of Covid-19. The patient was found to have larvae in their nares on 08/24/21. The was no documented oral care 08/24/21. The patient had documented oral care on 08/25/21 at 0038, 0228, 0549 and 2010.
The next documented oral care was on 08/26/21 at 0218 and 0325. There was no other documented oral care on 08/26/21.
Patient #8 was admitted on 07/31/21 with a diagnosis of Covid-19. The patient was to found to have larvae in their buccal cavity on 08/24/21. There was documented oral care on 08/24/21 at 2227. The patient had documented oral care on 08/25/21 at 0414. The patient had documented oral care on 08/26/21 at 0342 and 0700.
During an interview on 08/26/21 ending at 1150 Personnel #2 verified the above findings. Personnel #2 stated after the larvae were found the nursing staff was educated on the facility policy for oral care on ventilated patients.
The policy titled Oral Care Protocol revised 06/21 reflected..."Protocol...2. Intubated patients are provided oral care every 4 hours and more often as necessary..."
