Based on observation, interview and record review, the hospital failed to ensure the patient's right to receive care in a safe setting for 1 of 1 area (hallway), in that a basket of sheets and towels were observed in the hallway of Jackson 5 unit East wing during a tour of the hospital on 09/21/15.
On 09/05/15 at 6:00 PM Patient #10 had hung herself with a sheet.
Findings Included:
The Admission and Discharge Information form for patient #10 dated 09/09/15 at 7:42 PM reflected, "Although the patient was on suicide precautions, when the staff went and checked on the patient, they did not find her in her bed and found her pillow as if she was still sleeping in the bed. When they found that, the staff checked the bathroom and found that the patient was strangling herself with a sheet."
During the surveyor's tour of the hospital on 09/21/15 at 2:30 PM with hospital personnel #6, an uncovered, unmonitored laundry basket was found in the hallway of the Jackson 5 unit with sheets and towels on the East Hallway.
During an interview with hospital personnel #6, at the time of the tour, the charge nurse on duty was informed by personnel #6, to implement new procedures for retrieving linens.
The hospital Suicide Precaution Policy dated 01/2015 states, the hospital is to maintain a patient in a safe and therapeutic environment.
