MAINTENANCE OF PHYSICAL PLANT

Based on document review, observation, and interview, the hospital failed to ensure the condition of the hospital environment was maintained in a clean and safe manner for 3 of 7 patient care areas [Serenity Way (SW), Success Road (SR), and Journey Way (JW)] in 1 of 2 buildings (building 13).

Findings include:

1. Review of the policy titled Housekeeping Services, last revised 1/2018, indicated that general housekeeping on patient units was the responsibility of Nursing and Environmental Services. The policy also indicated that cleaning of patient rooms was done by unit staff or assigned housekeeper(s) and patients and that cleaning done by patients was supervised by staff.

2. On 1/24/22, beginning at approximately 1:45 PM, the following was observed during tour in building 13, of Serenity Way (SW), Success Road (SR), and Journey Way (JW) patient units:
On the SW unit, in the toilet of an unoccupied bathroom (room number not noted) was a live drain fly/gnat type insect. Dead appearing drain fly type insects were noted on the window seal.
On the SR unit, in the nurse's station 2 ceiling tiles were noted to have brown water type stains, neither appeared wet at that time. In patient room A233 no areas of the ceiling appeared to have leak damage or obvious signs of leaking. On the window sill, 2 dead appearing drain fly type insects and a dead appearing lady bug type insect were noted. In patient room A227, 10 or more drain fly type insects (most appeared dead and at least one flew away) were noted on/near the window sills. No obvious ceiling concerns were noted. In the day room a dead appearing house fly type insect was noted on the floor under a table and 4 more on the floor near a window. In another patient room (room number not noted), beside the patient bed/mattress on the floor was a pile, approximately 2"x 2" of dust type debris mixed with globs of hair and multiple used band-aide type bandages. A water type stain was noted on the ceiling over the patient bed area. In room B245, 4 drain fly type insects appeared dead lying on the window sill. In room B240 a dead appearing winged insect was noted on the window sill.
On the JW unit, in room B234 a dead stink bug type insect and a house fly type insect were noted on the floor. The floor edges were dirty appearing and covered with thick dust. All window sills were noted to have blackish colored dust along the edges and dead appearing drain fly type insects. Room B232 was noted to also have dead appearing drain fly type insects on the window sill and dust/dirt debris around the perimeter. In room B216, a live/moving drain fly type insect was noted.

3. On 1/24/22, beginning at approximately 3:00 PM, A1, Quality Manager, verified the patient units of building 13 were unclean/not well maintained and that both live and dead pests were observed. A1 indicated patients were encouraged to clean their own rooms, but it was the nurse's responsibility to ensure the rooms were kept clean.
