Based on findings from observation, and interview, hospital staff did not follow basic infection controls standards.

Findings include:

--Per observation on 2/12/15 at 2:45 pm, Environmental Services Staff #1 removed contaminated needles from an overfull sharps container with gloved hands and placed them in another sharps container. When asked why he/she did this, Environmental Services Staff #1 explained the sharps container was over flowing so he/she could not put the cap on. He/she removed the needles that were sticking out in order to place the cap on.

-- During interviews with Infection Preventionist #1 on 2/12/15 at 3:10 pm and the Director of Environmental Services on 2/12/15 at 3:40 pm, they acknowledged this is not acceptable and noted they have never observed any staff member doing this.
