Based on document review and interview, the facility did not ensure the completion of a respiratory inservice and competency review related to the use of the BiPap (Bilevel Positive Airway Pressure) machine and ventilator was documented for 3 of 14 Emergency Department (ED) Registered Nurses (RN), (Staff R, S and T, per diem ED RN's). Failure to ensure completion of competencies has the potential to negatively impact patient care.
Findings Include:
Review on 04/18/17 of Staff Education Files for Staff N through AA, ED RN's revealed no evidence to indicate Staff R, S and T completed the respiratory inservice and competency regarding the use of the BiPaP machine and ventilator that was provided to ED staff.
Interview on 04/18/17 at 1:00 PM with Staff A, RN-Quality Director, Staff B, RN-Director of Nursing, and Staff C, RN- ED Director, revealed that the competencies for Staff R, S and T were not added to their electronic education file and the paper competency could not be located.
