Based on observation, interview, and testing, the provider failed to ensure exits were readily accessible at all times. Three randomly observed marked exit doors (ambulance garage doors G210A, door 1200x, and stair enclosure door 1252) were equipped with magnetic locks. The doors were not equipped with signage that indicated how to open the magnetically locked doors in an emergency. Findings include:

1. Observation beginning at 8:00 a.m. on 7/24/12 revealed three randomly observed marked exit doors from the facility were equipped with magnetic locks: ambulance garage double doors G210A, door 1200x, and stair enclosure door 1252. There was no signage indicating how to open the magnetically locked doors in an emergency. Interview with the director of facilities confirmed that condition. He revealed the doors were delayed egress doors and could be opened even if locked. Testing the doors at the time of the observation confirmed they were delayed egress. The director of facilities said signage would be ordered and installed at the magnetically locked door locations as soon as possible.
