Based of record review and interview the facility failed to provide patient's with written notice for a grievance that was filed on behalf of each patient. Citing 2 out of 2 patients who's family filed called the hospital and filed a grievance after the patients discharge.
Findings:
Review of the grievance logs revealed that there was no evidence recorded on the logs regarding two grievances called to the hospital on behalf of patient # 1 and #2. The hospital had hand written notes concerning the complaints but failed to conduct an investigation of the complaints. In addition there was no evidence provided that the complaints were logged into the complaint system and no evidence the grievance committee reviewed the complaints and provide the complainants written follow of the findings.
Interview with the Patient Advocate on 04/02/2014 at 10:30 a.m. confirmed she had received a calls from family members of Patient # 1 and #2. She stated they both called complaining about the care the patients had received while in the hospital. She stated she showed her previous director the information and was told not to include either complaint on the complaint log. She confirmed the Director was no longer working for the hospital.
