Based on observation and staff interview on February 22, 2023 between approximately 0930 to 1230 hours the facility has failed to properly maintain fire/smoke barriers within the facility as capable of resisting the passage of smoke. This could result in the products of combustion traveling from one smoke compartment to another which would endanger the patients, staff, and/or visitors within the facility.
The findings include:
The Data room on ground floor has an open conduit.
The above was discussed and acknowledged by the facility staff.

Based on observation and staff interview on February 22, 2023 between approximately 0930 to 1230 hours the facility failed to restrict the use of extension cords and non-approved power strips in their facility. This could endanger patients, staff, and visitors in the facility due to the increased fire risk.
The findings include:
The Maintenance office has an extension cord plugged into a power strip.
Sleep room 2145A on 2nd floor has an unapproved multi plug adapter in use.

Based on observation and staff interview on February 22, 2023 between approximately 0930 to 1230 hours the facility has failed to maintain security of compressed gas cylinders. This could allow for the tampering with or damage to of cylinders, which could endanger patients, staff, and/or visitors.
The findings include:
Medical Gas storage room 1521A has nitrogen cylinders that are not properly secured.
