Based on observation and staff interviews it was determined the facility failed to meet the requirements of K-Tag 46.
This could place 195 residents at risk in the event of a fire or any other disaster requiring evacuation.
The findings include:
On 10-15-14 between 10:00 am and 11:30 am Observation revealed that there are no existing emergency lighting on the exterior of the building over entrance and exit doors.
These findings were confirmed by Staff M at the time of discovery.

The findings include:
On 10-15-14 between 10:00 am and 11:30 am Observation revealed that in various locations in A-wing and patient area corridors the fire sprinkler heads were down below ceiling level.
Also there is no signage to indicate location of Fire department connection for Fire Rescue.

Also, covers were not on electrical boxes, exposing wiring, in sprinkler riser room located near patient pharmacy.
