Based on review of six (6) of six (6) personnel records and staff interview, it was determined the facility failed to ensure that staff that are unvaccinated against COVID-19 are tested two (2) times per week, in accordance with facility policy.
Findings include:
Reference: Facility policy titled, "COVID-19 Vaccine Policy" states, "... Individuals who request and receive a medical or religious exemption must be COVID tested twice a week. ... ."
Upon interview on 2/23/22 at 12:45 PM, Staff #8 stated that staff that are unvaccinated against COVID-19 are required to test for COVID-19 twice per week. He/she stated the test results are input into an application, which is tracked.
Upon request, Staff #8 and Staff #10 were unable to provide COVID-19 testing results for unvaccinated staff. Staff #8 and #10 were unable to provide evidence that staff unvaccinated against COVID-19 are tested two (2) times per week.
