4. Inspection of Shepardson 6 revealed that the Fire door near room 636 is not closing and must be adjusted.

2. Inspection revealed that the Sprinkler system Pre-action valve cabinet was installed at the top of the MRI wing egress stairway located in the MRI wing of FAHC Main Campus.

2. Inspection of Shepardson 5 revealed that the Exit door at the west end of the corridor is on an automatic release set for 30 seconds, but the door is labeled with a 15 second delay.

FAHC ACC West Pavilion 5th Floor by room WP5-204 - Inadequate smoke detector coverage.
FAHC ACC West Pavilion Room 2nd Floor WP2-237 - Smoke detector less than 3 feet from supply air diffuser.
FAHC Garden Pavilion - Smoke Detector #M3-49 at entrance door less than 3 feet from supply air diffuser.
FAHC ACC Main Pavilion 4th Floor Room MP4-108 - Smoke detector missing.
FAHC Garden Pavilion near Room 2324 - Smoke detector less than 3 feet from supply air diffuser.

FAHC McClure 1 Inergen Fire Suppression System circuit breaker-lock missing. This was written up during the last two annual inspections and was not corrected.

2. Inspection of Baird 4 revealed that smoke detectors in corridors exceed standard spacing with as much as 74 inches between detectors. Additional smoke detectors must be provided.

3. Inspection of Shepardson 6 revealed that four smoke detectors on the unit are located within 1 foot of supply air grill and must be located at least 3 feet from the supply air. Also, the Fire alarm conduit needs approved fire stopping material in the wall next to room 637.

4. Inspection of Shepardson 5 revealed that the Fire Alarm panel shows lobby 401 smoke detector as dirty and needs cleaning. This detector had a solid light on, indicating a trouble.

5. Inspection of Shepardson 4 revealed a smoke detector near room 446 is located within 1 foot of supply air grill and must be located at least 3 feet from the supply air. Also, all smoke detectors in "On-Call Wing Bedrooms" are located within 1 foot of supply air grill.

b. O ' Brien Community Center in Winooski is missing a sprinkler head in the Aquatic Pool Closet.

1. Inspection revealed that there is frying being conducted in the Fanny Allen Campus Rehab Dining Rooms without a kitchen hood or fire suppression system.

2. Inspection revealed that the kitchen staff did not know how to manually activate the kitchen hood fire suppression system in the FAHC Fanny Allen Campus Cafeteria.

Room 451 is a small office and has a metal cabinet pressed against electrical outlet/cords. Outlet must be relocated away from cabinet.
