This Standard is not met as evidenced by:

Based on observation, the facility failed to provide corridors are separated from use areas by walls constructed with at least ½ hour fire resistance rating in accordance to NFPA 101 19.3.6.1, 19.3.6.2, 19.3.6.4, 19.3.6.5. This deficient practice had the potential of affecting one (1) of four (4) smoke compartments of the facility.

Findings include:

While inspecting corridor walls on 01/12/16 at 11:50 AM,observation revealed four (4) open penetrations in the corridor wall near the Psychiatric Stair Door on the 1st Floor.

The finding was acknowledged and verified by the Administrator during the exit interview on 1/12/16.

Based on observations the facility failed to provide the required 30 minute fire resistance rating for smoke barrier walls in accordance with 19.3.7.3, 19.1.6.3, 19.1.6.4. These deficient practices have the potential of affecting two (2) of four (4) smoke compartments of the facility.

Finding include:

While inspecting smoke barrier wall on 1/12/16, observation revealed the smoke barrier on the 1st floor near Conference Room had unsealed penetration from cables above ceiling.

The finding was acknowledged and verified by the Administrator during the exit interview on 1/12/16.
